Copyright: Provincia di Trento Home as the first place of care and telemedicine (Mission 6, Component 1, Investment 1.2)The Recovery and Resilience Plan supports the large-scale adoption of telemedicine solutions and supporting healthcare innovation, through the following measures:1. Home care as first point of assistance (Investment 1.2.1) - The goal is to increase the number of people treated in home care to 10% of the population over 65 through investment in hardware and increased service provision.2. Territorial Coordination Centres (Investment 1.2.2) - The planned investment concerns the establishment of 602 Territorial Coordination Centres (“Centrali Operative Territoriali”) that are intended to link and coordinate various territorial, social-health and hospital services, as well as the emergency-urgency network. The Territorial Coordination Centres will ensure the remote control of the devices provided to patients, support the exchange of information between health professionals, and constitute a reference point for caregivers and patient needs.3. Telemedicine to better support patients with chronic diseases (Investment 1.2.3) - The investment aims to (1) finance projects that enable remote doctor-patient interactions, in particular diagnostics and monitoring, (2) create a national platform for screening telemedicine projects (linked to Mission 6 Component 2 Investment 1.3) and (3) finance ad hoc research initiatives on digital health and care technologies.The investment is financed by the Recovery and Resilience Plan with EUR 3 billion.This project is part of this investment. The Territorial Operations Centre (COT) of Riva del Garda is part of a network of local healthcare facilities created to improve the efficiency and quality of healthcare and social services, in line with the National Recovery and Resilience Plan (PNRR). The COT acts as a coordination point for patients and families, managing integrated care pathways between hospital and territorial services, especially for people with chronic diseases or complex care needs. The COT of Riva del Garda facilitates the connection between the various healthcare actors, such as family doctors, hospital departments, emergency services and home care, ensuring a smooth transition between different levels of care. The facility also coordinates protected discharges, allowing patients to receive long-term care at home with adequate support. The PNRR has allowed the COT to implement new monitoring and telemedicine technologies, promoting constant monitoring of patient health and a rapid response in case of need. The digitalization of clinical data facilitates the secure sharing of information between professionals, improving the quality and timeliness of care. These tools allow for better management of chronic or fragile patients, reducing the need for travel and hospitalization and promoting sustainable home care. The Riva del Garda COT model, which is not directly accessible to citizens but can be activated through general practitioners, represents an example of inclusive local healthcare. Thanks to a coordinated approach, the COT responds to local needs and reduces the burden on hospitals, helping to improve the quality of life and resilience of the healthcare system.The location of the Riva del Garda COT is the Health Services Centre in via A. Rosmini n.5/B.The spaces planned for the Territorial Operations Centre are:- room used as the Operations Room, an area in which the workstations are located;- support rooms for secretarial, administrative, archive, storage and personal relaxation equipped with furniture and equipment suitable for carrying out the activity shared with the remaining functions present in the structure and sized according to the volumes of activity;- changing rooms for staff, toilets divided by gender in a number proportional to the staff working there;- technical rooms for telephone, radio and IT installations;- support/storage room for maintenance and cleaning services;- meeting rooms, areas to be used for staff training/training also suitable for distance learning;- parking areas commensurate with the staff working in the Operations Centre.This project is financed by the Recovery and Resilience Facility with EUR 17000 ReferenceM6C1Project locations Italy EU contribution€17 000 Project websitehttps://www.provincia.tn.it/Documenti-e-dati/Progetti/Casa-come-primo-luogo-di-…
