Copyright: Provincia di Trento Home as the first place of care and telemedicine (Mission 6, Component 1, Investment 1.2)The Recovery and Resilience Plan supports the large-scale adoption of telemedicine solutions and supporting healthcare innovation, through the following measures:1. Home care as first point of assistance (Investment 1.2.1) - The goal is to increase the number of people treated in home care to 10% of the population over 65 through investment in hardware and increased service provision.2. Territorial Coordination Centres (Investment 1.2.2) - The planned investment concerns the establishment of 602 Territorial Coordination Centres (“Centrali Operative Territoriali”) that are intended to link and coordinate various territorial, social-health and hospital services, as well as the emergency-urgency network. The Territorial Coordination Centres will ensure the remote control of the devices provided to patients, support the exchange of information between health professionals, and constitute a reference point for caregivers and patient needs.3. Telemedicine to better support patients with chronic diseases (Investment 1.2.3) - The investment aims to (1) finance projects that enable remote doctor-patient interactions, in particular diagnostics and monitoring, (2) create a national platform for screening telemedicine projects (linked to Mission 6 Component 2 Investment 1.3) and (3) finance ad hoc research initiatives on digital health and care technologies.The investment is financed by the Recovery and Resilience Plan with EUR 3 billion.The Territorial Operations Centre (COT) of Pergine Valsugana is part of an initiative aimed at improving healthcare coordination for patients with chronic and complex needs, particularly the elderly and frail. The COT functions as an integration hub for healthcare and social services, connecting hospitals, family doctors, care facilities and social services, to ensure more effective continuity of care and reduce fragmentation and waiting times. One of the distinctive aspects of the COT of Pergine is the use of advanced digital technologies for sharing and constantly updating healthcare data, allowing timely interventions based on real-time clinical data. This digital system supports the rapid and effective management of emergencies and health changes, reducing repetitive interventions. In addition to clinical care, the COT provides social and psychological support thanks to a multidisciplinary team that includes doctors, nurses, social workers and specialists. The family is involved in the decision-making process, for long-term support. The COT aims at home care, which is essential to maintain the dignity of frail patients and reduce hospital admissions. Collaboration with volunteers and community resources strengthens healthcare integration, promoting a care model based on proximity and sustainability. This approach ensures a continuous and personalized support system, enhancing the well-being of the local community.The project included several interventions to improve the functionality and efficiency of the building:- the floor was redone to improve insulation and facilitate the passage of systems- the internal organization of the spaces was renewed, since the structure of the area allowed a complete reorganization without structural interference;- at ceiling level, the project involved the demolition and replacement of the existing false ceiling with a double structure to protect the structural elements and mask the systems;- the windows were replaced with more energy-efficient models, maintaining the original geometry and providing new PVC shutters;- the old ventilation ducts on the facade were removed.The systems have been completely renovated:- the electrical system has been redone and powered by an updated distribution system with a new electrical panel;- general and emergency lighting has been optimized with the installation of LEDs and standard lighting points;- the fire detection system has been updated with repositioned and integrated smoke detectors;- a new air conditioning system has been implemented with renewed pipes and fan coils installed in the rooms and in the corridor, to ensure a comfortable environment;- the water and sanitation systems have been updated.This project is financed by the Recovery and Resilience Facility with EUR 800000 ReferenceM6C1Project locations Italy EU contribution€800 000 Project websitehttps://www.provincia.tn.it/Documenti-e-dati/Progetti/Casa-come-primo-luogo-di-…
